In my view, the fastest way to check an NEMT site is to follow the rider’s path and verify the numbers that matter: 36-inch routes, 32-inch door clearance, 1:12 ramp slope, 60-inch turning space, and marked parking and loading areas. ADA-Compliant NEMT Facility: Key Numbers at a Glance

Start at the curb or parking space, then follow the exact path a rider takes to reach the entrance. Check the route used most often first. After that, review the other spaces that support it.
There should be one continuous accessible route from parking, the sidewalk, or the nearest transit stop to the entrance, with no steps.
Walk the route with a tape measure. The path needs to be at least 36 inches wide, though 32 inches is allowed at pinch points. If two users need to pass each other, the route must provide 60 inches of width at that spot. As you walk, look for cracked pavement, utility boxes, loose signs, or anything else that shrinks the path below the minimum.
Slope is another big one. Any ramp on the route cannot be steeper than a 1:12 ratio - that means 1 inch of rise for every 12 inches of run. And if there’s a rise of more than 0.5 inches, it needs a ramp, curb ramp, or elevator. Check the walking surface too. It should stay slip-resistant, especially after rain or after years of wear.
For NEMT sites, there’s one point you can’t miss: if riders are picked up or dropped off at a side or rear entrance, that entrance needs the compliant accessible route.
Once the route checks out, move on to parking size and markings.
Provide 1 accessible parking space for every 25 total spaces.
A standard accessible space must be at least 8 feet wide and include a 5-foot access aisle next to it. A van-accessible space needs an 8-foot access aisle so a ramp can deploy safely. The loading zone should be level, slip-resistant, and set up with a marked access aisle so transfers can happen safely. It also needs a clear ground space of 30 × 48 inches for a wheelchair during transfer.

Once the exterior route and loading zone check out, head inside. Follow the same path a rider would take: from the front door to reception, waiting areas, and restrooms. Then review the waiting area, counters, restrooms, and signs.
Each accessible entrance must provide at least 32 inches of clear width when the door is open to 90 degrees. At every door, check these items:
Interior corridors that link the entrance to reception, waiting areas, service areas, and restrooms must stay at least 36 inches wide the whole way. If two wheelchair users may need to pass, that spot must allow 60 inches of width.
Watch for protruding-object hazards like fire extinguishers, bulletin boards, and wall signs. Mounted items should stay within reach ranges: 15 to 48 inches for forward reach and 9 to 54 inches for side reach.
If your facility has more than one level with service areas, those spaces must connect by ramps, curb ramps, or elevators. If a broker contract or state Medicaid program sets a stricter rule than the federal ADA baseline - such as wider door clearances or more frequent inspections - follow the stricter rule.

Keep the accessible route from the entrance to seating, reception, and exits free of obstacles. That means no chairs drifting into the path, no trash cans parked in the way, and no charging stations cutting off access.
Provide one 60-inch turning circle and one 30 × 48-inch clear floor space. Those two measurements matter because they give wheelchair users room to move, turn, and stop without a hassle.

Next, check the places where riders interact with staff and use the restroom. Provide one accessible counter section within 15 to 48 inches of reach. If the space has only a tall counter, add a lower section that falls within that reach range.
Restrooms should include a 60-inch turning space, grab bars with a 1.25- to 1.5-inch diameter, and slip-resistant flooring. Sink controls, soap dispensers, and paper towel dispensers should sit within the 15- to 54-inch side reach range.
A small detail can make a big difference here. If a rider has to stretch, twist, or ask for help just to reach soap or paper towels, the room isn't doing its job.
Finish by checking the signs that guide riders to the entrance, parking, and accessible paths. Use high-contrast directional signs for accessible routes. Exterior business signs should use letters at least 3 inches high in a contrasting color.

Physical access is only part of the job. Riders also need staff who can communicate clearly, help in a safe way, and respond well during an emergency.
