An incident report is the formal record of anything that goes wrong on or around a trip: a vehicle accident, a rider fall or injury, a medical event, a securement failure, a complaint, an altercation, property damage or a lift malfunction. Broker contracts require reporting within a fixed window, often 24 hours, and insurers require prompt notice of anything that could become a claim.
Late or missing incident reports are a contract violation and, in litigation, evidence of poor management. A consistent process, with drivers trained to report immediately, photos and GPS data attached, and management review within the day, protects riders, drivers and the business.
