What is a trip log?

A trip log is the documentation for a single trip: rider name and ID, authorization number, pickup and drop-off addresses, scheduled and actual times, odometer or GPS mileage, level of service, driver and vehicle identifiers, and the rider's signature or an accepted alternative. Paper trip sheets are still used, but most payers now accept or require electronic logs generated by the driver app.

Why trip logs matter for NEMT

The trip log is your evidence in an audit. State Medicaid programs and brokers audit years back, and a trip you cannot document is a trip you repay, with penalties for patterns. Electronic logs with GPS breadcrumbs and timestamped signatures survive audits; handwritten sheets with rounded times do not.
