A case manager coordinates care for patients with complex needs, working for a health plan, hospital, clinic or community program. They assess needs, arrange services, track appointments and help remove barriers to care, transportation chief among them. Health plan case managers can often authorize NEMT trips directly or through the broker on a member's behalf.
Case managers are referral sources and problem solvers. They know which members miss appointments because of transportation and can direct standing orders to providers who perform. Building relationships with case managers at the health plans and clinics you serve turns your company from an interchangeable broker vendor into the provider they ask for by name.
