What is a discharge planner?

A discharge planner is a hospital case manager, nurse or social worker responsible for arranging a patient's safe transition out of the hospital: home health, equipment, follow-up appointments, placement in a rehab or nursing facility, and transportation. Discharge planners work under pressure to free beds and need transport that can be booked quickly and shows up on time.
