What is a skilled nursing facility?

A skilled nursing facility (SNF) is a licensed nursing home that provides 24-hour nursing care and rehabilitation services, either for short-term recovery after a hospital stay or for long-term residents. SNF residents frequently need transportation to dialysis, specialist appointments, wound care, imaging and hospital visits, and most cannot travel by ordinary means.

Why SNFs matter for NEMT

A single SNF can generate dozens of trips a week, largely wheelchair and some stretcher, with facility staff handling scheduling. Facilities value reliability over price because a missed trip means a missed treatment and paperwork for them. Direct facility contracts, or serving the facility's residents through the broker, are both viable; either way the facility's transportation coordinator is your customer.
