What is a home health agency?

A home health agency provides skilled nursing, physical and occupational therapy, and aide services in a patient's home, typically after a hospitalization or for a chronic condition. Home health patients are by definition limited in mobility, and while clinical care comes to them, specialist visits, imaging, labs and dialysis still require a trip out of the house.

Why home health agencies matter for NEMT

Home health nurses see transportation gaps first-hand and can recommend a provider to patients and families, making agencies an efficient referral channel for private-pay and Medicaid trips. The trips are often door-through-door and wheelchair level, from single-family homes with steps, which suits providers with well-trained drivers. A short introduction to the agency's clinical managers, with your service levels and coverage area, is usually all it takes to start.
